This order form is only to be used for one of the following purposes:

• Ordering extra pieces for spares or replacements. We recommend adding an additional 5% to large orders in case a tile gets worn or damaged.
• If you know exactly how many tiles you need to make a unique design.
• You want a specific size mat that is not listed in our standard sizes.

• 2 detachable edge pieces are included with each 2' x 2' inside tile.

• Minimum order is 4 tiles. Orders for less than 4 tiles will be voided.
